About Your Initial Evaluation (Visit):
The In Touch Physical Therapy and Sports Medicine Staff want your visit to be as comfortable and enjoyable as possible. Education is a key component in keeping with our mission of providing the highest quality of care that encompasses each patient’s individual needs. The information below should help you prepare physically and mentally for your Initial Evaluation and subsequent Treatment Sessions. When to Come:
For your Initial Evaluation (Visit), we recommend you arrive 30 minutes before your scheduled appointment with your therapist to allow sufficient time to complete the necessary paperwork. If you have already completed the necessary paperwork, we ask that you arrive 15 minutes before your scheduled appointment with your therapist to allow sufficient time to complete the registration process. We agree that paperwork is frustrating and tedious, but it is necessary for all insurances and important for your Physical Therapist to have the information to evaluate and assess your current condition. You may Download the necessary Forms (Demographics, Patient Information, Financial Agreement) located under the Forms Section on our website. An In Touch Physical Therapy and Sports Medicine Staff Member will give you a courtesy call the day before your initial visit, or on the Friday prior to a Monday appointment, to confirm your appointment and arrival time.
What to Bring:
Please bring your Photo ID, Insurance Card, Medication List, Doctor’s Referral and any additional Medical / Diagnostic Documents (X-rays, MRI’s, CT Scans, etc.). If you had any Medical / Diagnostic Tests or Imaging performed, but you don’t have the documents, please let an In Touch Physical Therapy and Sports Medicine Staff Member know when you arrive. We will have you sign a Medical Release Form, fax it to the appropriate medical office and have the documents faxed to our office.
What to Wear:
Please wear comfortable clothes, which allow movement and accessibility to the involved area(s) of your body. For example: If you are being seen for a shoulder issue, for women a Tank Top or Jog / Sports Bra would be appropriate.
What to Expect:
Initial Evaluation (Visit) and Treatment Session
Our staff is attentive and caring. Your experience should be comfortable and individualized to your personal needs. An In Touch Physical Therapy and Sport Medicine Staff Member will assist you throughout your physical therapy treatment session. You will be greeted when you arrive and directed to a patient room or a treatment area to await your Physical Therapist. Your visit will consist of a comprehensive evaluation and treatment, time permitting, by your Physical Therapist. Your Physical Therapist will review your paperwork and medical documents and discuss your past medical history and current condition / issue. They will then perform a thorough Musculoskeletal Assessment pertinent to your condition / issue, including appropriate Special Tests and Measures. Treatment will usually be performed at the time of your Initial Evaluation, time permitting, and if deemed appropriate by your Physical Therapist. Modalities may be administered at the end of the session, if deemed medically necessary by your Physical Therapist. Your time with the therapist will be about 45-60 minutes with 15-20 minutes at the end for any modalities.
Please Note: It is not unusual to experience a temporary setback or exacerbation in your condition following the Initial Evaluation and Treatment Session. The process of performing a thorough Musculoskeletal Assessment and the appropriate Special Tests and Measures, as well as conducting initial treatments, may lead to some acute inflammatory processes. This should subside and will become less of an issue during subsequent treatment sessions.
Standard Treatment Sessions
Your treatments following the Initial Evaluation (Visit) will be by your primary Physical Therapist or a Physical Therapist Assistant to maintain your continuity of care as much as possible. Sessions will consist of 30-45 minutes of direct, one-on-one, individualized patient care, followed by 15-20 minutes of modalities, if deemed medically necessary by your Physical Therapist / Physical Therapist Assistant. Your treatment frequency, session duration, treatment techniques, and modalities may change as deemed appropriate and necessary by your Physical Therapist / Physical Therapist Assistant, according to your condition and needs during each therapy session.
